                   CALIFORNIA STATE POLYTECHNIC COLLEGE
                     APPLICATION FOR CLUB RECOGNITION


In order that proper records may be kept and proper classifications be made
of all clubs and organizations, will you please fill in the appropriate blanks and
attach three copies of your proposed constitution. Upon approval, you will be
given a copy of the approval sheet and your constitution. Approval may be revoked
by the S. A. C. or the administration if there is not strict adherence to your con-
stitution or to the purposes and activities as outlined in your application. It
will be the duty of the club considered to maintain a current constitution by pro-
viding written copies of changes to the Dean of Student Welfare and the President
of the Student Body. The student or graduate manager will handle the funds for your 
organization.
